19 eateries failed Pima County health inspections in May

Nineteen food service establishments failed Pima County Health Department inspections in May.
All of them went on to pass follow-up inspections.
Here’s what you need to know:
May restaurant inspections: Oregano’s Pizza Bistro
UpdatedHistory: Good and excellent scores for several years but was placed on probation May 8.
Health inspectors found food debris and residue on dinner plates that were supposed to be clean during a May 8 inspection at Oregano’s Pizza Bistro, 100 W. Orange Grove Road.

May restaurant inspections: Bisbee Breakfast Club
UpdatedHistory: Good ratings since 2017 but received a needs improvement rating May 9 and failed initial re-inspection May 10.
What the inspector saw: Ham, cheese, potatoes and applesauce at unsafe temperatures; clean utensils stored in dirty container; clean cutting boards stored on dirty shelf; food handlers did not wash hands and put on new gloves after changing tasks.
Follow up: Passed a second re-inspection May 13.
Comments: Owner Terry Kyte said a refrigeration problem was quickly repaired. Food was transferred to a different unit and customers were not affected, he said.
May restaurant inspections: Broadway Proper Retirement Community
UpdatedHistory: Mainly good and excellent scores for several years but received a needs improvement rating April 29 and failed initial re-inspection May 6.
What the inspector saw: Steak, sour cream and cooked chicken stored at unsafe temperatures; hamburger was undercooked.
Follow up: Passed a second follow-up inspection May 16.
Comments: Declined to comment.
May restaurant inspections: Bruegger’s Bagel Bakery
Updated
History: Mainly good and excellent ratings for years until 2019. Since January, three of six health inspections had negative findings, including a May 22 needs improvement rating and a failed re-inspection May 23.
What the inspector saw: Ham, cheese mayonnaise and cream cheese stored at unsafe temperatures; cockroach crawling on a sink; “encrusted food debris” in refrigerator; some refrigerated foods had no use-by dates; trash and food debris on floor in food preparation area.
Follow up: Passed a second re-inspection June 3.
Comments: Declined to comment.
May restaurant inspections: Cafe 54
UpdatedHistory: Good and excellent scores for years but received a May 10 needs improvement rating and failed a May 13 follow-up inspection.
What the inspector saw: Mashed potatoes, deli turkey and cut melon stored at unsafe temperatures.
Follow up: Passed a second re-inspection May 14.
Comments: Chef Kevin Powers said a faulty refrigeration unit was quickly repaired and did not affect customers.
May restaurant inspections: Circle K
UpdatedHistory: Good and excellent scores until 2017. Since then, four of seven health inspections had negative findings including a May 7 needs improvement rating followed by two failed follow-up inspections May 17 and 28.
What the inspector saw: Sandwiches, burgers and burritos stored at unsafe temperatures; two hand-washing sinks had no paper towels; crackers stored next to insecticide and detergent.
Follow Up: Passed a third-follow-up inspection June 7.

May restaurant inspections: Circle K
UpdatedHistory: Good ratings throughout 2018 but was placed on probation May 7.
What the inspector saw: Hamburgers and cheeseburgers, club sandwiches and breakfast burritos were stored at unsafe temperatures — the store’s third temperature violation in a row; boxes of food products were stored on floor of walk-in refrigerator.
Follow up: Passed re-inspection May 17.

May restaurant inspections: DD’S Baja Seafood Grill
UpdatedHistory: Mainly passing scores in 2018 but was placed on probation May 1.
What the inspector saw: Raw oysters were not tagged to indicate supplier and source waters; four varieties of salsa were stored at unsafe temperatures; employee cut up limes with bare hands; hand-washing sink blocked.
Follow up: Passed re-inspection May 13.
Comments: Owner Jose Montano said new employees were retrained and problems have been corrected.
May restaurant inspections: Doña Alicia Mexican Food

History: Good and excellent scores since late 2017, but received a needs improvement rating May 15 and failed re-inspection May 16.
What the inspector saw: Beans, salsa, chicken and shredded beef stored at unsafe temperatures; hand-washing sink had no paper towels; no test strips for sanitizing solution.
Follow up: Passed a second re-inspection May 22.
Comments: Owner Rosania Quinones said a faulty refrigeration unit has been fixed.
May restaurant inspections: El Minuto Cafe
UpdatedHistory: Mainly good scores for several years but was placed on probation May 13.
What the inspector saw: Tamales, cheese, shredded chicken and ground beef stored at unsafe temperatures; no use-by dates on refrigerated foods; food debris on “multiple food contact surfaces.”
Follow up: Passed a follow-up inspection May 23.
Comments: Manager Zulema Salinas said problems were quickly corrected.
May restaurant inspections: El Molinito

What the inspector saw: Chicken, beans, shredded beef, salsa and enchilada sauce stored at unsafe temperatures; cockroach found inside a cooking pot stored with clean dishes; hand-washing sink lacked soap, paper towels and hot water; kitchen equipment was not sanitized.
Follow up: Passed a second re-inspection June 13.
May restaurant inspections: El Patio
UpdatedHistory: Good and excellent ratings for several years but was placed on probation May 6.
What the inspector saw: Beef, beans, chorizo, tamales and cheese stored at unsafe temperatures; insecticide stored in kitchen; management not certified in food protection management.
Follow up: Passed re-inspection May 16.

May restaurant inspections: Five Points Market & Restaurant
UpdatedHistory: Mainly good and excellent scores since 2016 but was placed on probation May 21.
What the inspector saw: Pork, chicken salad, bean and ham soup and blue cheese crumbles stored at unsafe temperatures — a repeat violation; cooking equipment stored as clean had food debris — also a repeat violation.
Follow up: Passed re-inspection June 3.
Comments: Could not be reached for comment. No one answered the phone during business hours and there was no voicemail system.
May restaurant inspections: Jason’s Deli
UpdatedHistory: Good and excellent scores until mid-2017. Since then, four of 10 inspections had negative findings including an April 23 needs improvement rating and a failed re-inspection May 3.
What the inspector saw: Tuna salad, potato salad and coleslaw stored at unsafe temperatures; no sanitizing solution in equipment-washing machine; sewage overflow in sink.
Follow up: Passed a second re-inspection May 13.
Comments: Declined to comment.
May restaurant inspections: Jerusalem Market
UpdatedHistory: Licensed for food service since April, the site had negative findings in three of four inspections in May. They include a May 1 needs improvement rating and two failed re-inspections on May 2 and 15.
What the inspector saw: Cheese, yogurt and kefir stored at unsafe temperatures; no sanitizing solution in sanitizing sink; hand-washing sink had no paper towels; manager not certified in food protection management.
Follow up: Passed a third re-inspection May 28.
Comments: Manager George Camacho said problems have been corrected.
May restaurant inspections: Miss Saigon
UpdatedHistory: Has failed four of seven inspections in the past 14 months. Most recently was placed on probation May 16 and failed two re-inspections May 28 and June 7.
What the inspector saw: Beef, chicken, tofu, bean sprouts and noodles stored at unsafe temperatures; rodent droppings in storage area; cockroaches “throughout the facility;” dishwasher had no sanitizing agent; heavy grease and food debris buildup on floors and walls throughout the premises.
County health inspectors found numerous problems, including cockroaches “throughout the facility,” at Miss Saigon restaurant, 1072 N. Campbell Ave.

May restaurant inspections: Nico’s Restaurant
UpdatedHistory: Good scores until 2017, since then four of seven inspections had negative findings. Most recently was placed on probation May 14 and failed re-inspection May 24.
What the inspector saw: Ham, fish, chicken and chorizo stored at unsafe temperatures; some foods had no use-by dates — a violation noted twice before; no test strips for sanitizing solution — a repeat violation; lettuce was washed in a hand-washing sink.
Follow up: Passed a second re-inspection June 3.
Comments: Manager Oyuky Garcia said problems have been rectified.
May restaurant inspections: Ritz-Carlton Dove Mountain
UpdatedHistory: Good and excellent ratings since 2016 but was placed on probation May 2.
What the inspector saw: Clean cooking utensils store in dirty containers; documentation lacking for shellfish sales; improper cooling procedures for cooked food — a repeat violation; hand-washing sink lacked hot water; dishwasher not functioning properly.
Follow up: Passed re-inspection May 14.
Comments: The problems inspectors identified “were promptly addressed to the full satisfaction of the Health Department,” marketing director Tim Anderson said.
May restaurant inspections: Wings Over Broadway
UpdatedHistory: The current food service permit was issued in August 2018. The restaurant received an excellent rating in February but was placed on probation May 23.
What the inspector saw: Chicken wings, cheese, coleslaw mix and ranch dressing stored at unsafe temperatures; wiping cloths not sanitized; hand-washing sink had no soap or paper towels; no disclosure of risk of foodborne illness from undercooked foods.
Follow up: Passed re-inspection June 4.
