A midtown senior living facility received a β€œprovisional” rating during a February health inspection, but passed its follow-up inspection 10 days later.

Villa Maria Care Center, 4310 E. Grant Road, logged six critical violations during a Feb. 2 inspection by the Pima County Health Department, documents show.

β€œEmployee placed a garbage pail lid on pail and switched to handling food without first washing her hands,” the inspection report says. β€œAnother employee moved a garbage pail with gloved hands and returned to food preparation.”

The inspector also cited the facility because the hand sink in the dish room was blocked by carts and piled with dirty kitchen utensils.

Other violations included employee beverages stored on shelves next to food, no sanitizer in the dishwashing machine, and a dish sink that drained directly into a sewer line, according to the inspection report.

β€œDishware stored as clean stored on unclean surfaces in service line area,” the inspector noted in the report. β€œMillwork shelving dirty, stainless shelves under equipment soiled with food debris and rodent droppings. Kitchen area utensils stored as clean have dried food debris on surfaces.”

The inspector also found evidence of a rodent infestation in multiple areas of Villa Maria’s kitchen and serving areas.

β€œ(Mouse feces) noted in serving area, on shelves storing cleaned dishes, under cook line, around refrigerator, in employee break area next to kitchen office and on kitchen cart in dishwashing room,” the report says.

The facility was instructed to clean all affected areas and show evidence of an β€œaggressive pest control program targeting rodents.”

The facility passed a follow-up inspection on Feb. 12.

A representative of the facility did not respond to a request for comment.

Villa Maria had a good track record with the Health Department since 2000 and had received β€œexcellent” ratings on its last 10 inspections.
