As part of an ongoing series about patient safety, the Arizona Daily Star wants to hear from you to guide our research.

The Star, in partnership with ProPublica, is gathering stories about patient harm. If you or a loved one has experienced a medical error, we encourage you to participate. All responses are considered confidential unless the patient says otherwise.

The information you enter into an online questionnaire will be submitted to and stored with ProPublica, which is an independent, nonprofit newsroom based in New York City that produces investigative journalism, including an ongoing investigation into patient harm.

ProPublica is sharing the information with us. It is subject to the ProPublica privacy policy. With your permission, you may be contacted by a reporter to follow up on your responses.

We’d like to engage as many community members as possible to shine a light on patient harm. Ideally, this yearlong project will encourage public dialogue on an important issue that continues to cause pain and suffering for Americans.
